The “OMR Application Form” and the “Information Brochure” can also be obtained by Post from the JAM Office of IIT Kanpur by making a written request along with the Demand Draft of Rs. 1050/- for General category Male candidates, Rs. 950/- for General category Female candidates and Rs. 550/- for SC/ST and PD candidates, drawn in favour of “Organising Chairman, JAM-2008”, IIT Kanpur, payable at SBI, IIT Kanpur (Branch Code: 1161), and a self-addressed slip (11.0 cm x 6.0 cm). This fee includes Rs. 50/- as postal charges. Such requests must reach the JAM office, IIT Kanpur, latest by December 26, 2007. The application material shall be dispatched by Registered Post. IIT Kanpur, however, does not take any responsibility for the late delivery of application material, due to postal delay or loss of documents in transit. Once an Application Form has been purchased either from the Bank or by post from the JAM Office, IIT Kanpur, any claim for refund of the fee, for any reason what soever, will not be entertained nor can this fee be held in reserve for future.

Candidate must send the duly filled in OMR Application Form or completed Online Registration Form, as the case may be, along with all necessary enclosures indicated, to the following address:
